Navigating patient safety with HSSIB and the RCP
This is Medical Care - driving change webinar. The HSSIB team will introduce the organisation’s role and provide insights into their work. There will be opportunity for discussion about related safety concepts and investigation.
The Health Services Safety Investigations Body (HSSIB) came into operation on 1 October 2023. HSSIB is an independent arm’s length body of the Department of Health and Social Care with the remit of investigating patient safety concerns across the NHS and in the independent sector in England. HSSIB’s investigations do not apportion blame or liability and result in recommendations to national bodies to help improve patient safety.
This patient safety resource has been supported by a grant from Becton Dickinson (BD). They have had no influence over the production or content of these materials.

Deinniol Owens
Deputy Director of Investigations
Deinniol qualified as a solicitor in 2008. He has worked with the Parliamentary and Health Service Ombudsman, investigating complaints about NHS care and treatment.
Deinniol joined HSSIB in January 2018 and has since gone on to develop his interest in human factors, completing an MSc in Patient Safety and Clinical Human Factors at the University of Edinburgh and becoming a chartered member of the Chartered Institute of Ergonomics and Human Factors. He has also completed the NHS Leadership Academy Nye Bevan programme in executive leadership and is currently studying for a PhD considering how simulation is used in the training of healthcare safety investigators at Staffordshire University.
Dr Nick Woodier
Senior Safety Investigator
Nick is a doctor with a background practicing emergency medicine. In response to his growing interest in safety science, he redirected his career and is now a human factors specialist with chartership through the Chartered Institute of Ergonomics and Human Factors.
Nick joined HSSIB in 2019 following several years working in the NHS where he held various roles leading safety investigations and governance programmes.
Nick’s specialist areas include investigation methodologies and tools, qualitative research, and supporting people after traumatic events. He holds a master’s degree in human factors and was awarded his PhD by the University of Nottingham for research into safety in healthcare and other safety-critical industries.
