Moving home can be a stressful time. Along with packing up your belongings, you may also need to transfer ongoing osteoporosis care to a new healthcare provider. This resource is designed to support both patients and caregivers by helping you collect all the essential information in one place, making the transition as smooth and simple as possible.
Background
If you have osteoporosis, it’s important to have a continuous and well-coordinated care plan. One of our patient carer panel members, Patricia Munn, has first-hand experience of moving home and transferring her care to a different NHS trust. While the process came with its challenges, it also gave her valuable insight into what can help make it easier.
This resource, created in collaboration with Patricia, is designed to simplify the process for you. It includes a practical list of information you may need to share with your new healthcare team - helping to make your transition as smooth and stress-free as possible.
This resource consist of two parts:

- An accompanying video with our clinical lead Kassim Javaid and Patient Carer Panel member Patricia Munn.
