Information for employing bodies who are wanting to fill a vacancy with an MTI doctor or who have already filled a vacancy with an overseas candidate. 

The Medical Training Initiative (MTI) is a mutually beneficial scheme that provides international medical graduates (IMGs) with the opportunity to work and train in the UK, while giving hospitals a high-quality, longer-term alternative to using locums to fill rota gaps. The Trust can benefit greatly from the scheme, by gaining access to a diverse pool of talented medical professionals who can help address staffing shortages and bring new perspectives to patient care.


The overall aim of the scheme is for the IMGs to receive high-quality training in the UK for a maximum of twenty-four months, after which they return to their home countries to put into practice the skills that they have learned and help improve healthcare systems globally.


The Royal College of Physicians MTI scheme is here to assist with the application process, including facilitating General Medical Council (GMC) registration for the candidates and a Government Authorised Exchange (GAE - previously Tier 5) visa if required.

  • Administrative support when applying for the Government Authorised Exchange visa, if required.
  • Guidance on creating job descriptions with adequate educational components, study leave opportunities and supervision for the MTI doctor.
  • Guidance for how best to support your MTI doctor when in post.
  • Opportunities to build links with international hospitals and countries seeking to send their trainees for valuable NHS experience.

The ‘Global supervision pack: Medical training initiative (MTI)’ is a comprehensive guide designed to support supervisors of international medical graduates (IMGs) working in the NHS through the MTI programme. ​It provides essential information on securing a post, the experience and requirements for MTI doctors, and the support offered by the Royal College of Physicians (RCP). ​The document outlines onboarding, induction, supervision, and pastoral support processes, ensuring a smooth transition and successful integration for IMGs into the UK healthcare system. It also includes guidance for NHS trusts on how to best support MTI doctors throughout their placement.
