As part of the Royal College of Physicians’ (RCP) focus on medication safety, we have produced a checklist for patients to help to ensure they have the information they need about their medication at hospital discharge.
We believe that patients are partners in their care. We have worked alongside our Patient Carer Network (PCN) to produce this checklist which we hope supports patients and their carers to best understand their medicines at discharge. The checklist takes the form of a series of questions to help them discuss their medication with healthcare professionals. The checklist also explains the key resources patients are given at discharge.
The checklist can be downloaded and printed and will be a useful resource for hospitals and patient charities to share with their patients as a mechanism to support patient safety by reducing medication errors at discharge.
