Long-term neurological conditions (LTNCs) comprise a diverse set of conditions resulting from injury or disease of the nervous system that will affect an individual for life. People in the UK living with a LTNC make up 19% of hospital admissions.
These guidelines build on the Quality Requirements in the National Service Framework (NSF) for LTNCs and aim to improve the quality of care offered to people with LTNCs by promoting effective collaboration between neurology, rehabilitation and palliative care teams.
This guideline is intended to inform doctors and healthcare professionals involved in the long-term support, rehabilitation and palliative care of people in the later stages of long-term neurological conditions. In many instances the guideline will also be useful for providers and purchasers of neurology, rehabilitation and palliative care services and other healthcare professionals including nurses and GPs.
