RCP collaborates on digital transformation project to improve patient services in Wales

Transfer of Care (TROCAR) is a digital transformation project which aims to improve the quality, speed and usefulness of information sent to GPs when patients are discharged from hospital. This new system will be introduced in Morriston hospital’s ward D following consultation with medical staff, GPs, hospital managers and patients, and will eventually be rolled out across the health board.

The project is being led by Dr David Price, associate unit medical director for medicine and consultant nephrologist, Dr James Chess at Morriston hospital, and by Professor John Williams, professor of health services research at SUMS and director of the Health Informatics Unit at the RCP.

This is the first project to be undertaken as part of a collaboration between the RCP, ABMUHB and SUMS. It will bring together complementary skills and experience with the shared goal of improving the safety and quality of services and care for the local population.

  • The Health Informatics Unit at the RCP is promoting and supporting quality improvement through digital transformation.
  • Swansea University Medical School supports operational clinical system development and use, and is a founder member of the Farr Institute which is dedicated to using data from health and social care to inform service developments and improve outcomes for patients and citizens.
