The Royal College of Physicians (RCP) is collaborating with Abertawe Bro Morgannwg University Health Board (ABMUHB) and Swansea University Medical School (SUMS) to improve the quality and safety of services provided by the health board to the local population.
Transfer of Care (TROCAR) is a digital transformation project which aims to improve the quality, speed and usefulness of information sent to GPs when patients are discharged from hospital. This new system will be introduced in Morriston hospital’s ward D following consultation with medical staff, GPs, hospital managers and patients, and will eventually be rolled out across the health board.
The project is being led by Dr David Price, associate unit medical director for medicine and consultant nephrologist, Dr James Chess at Morriston hospital, and by Professor John Williams, professor of health services research at SUMS and director of the Health Informatics Unit at the RCP.
This is the first project to be undertaken as part of a collaboration between the RCP, ABMUHB and SUMS. It will bring together complementary skills and experience with the shared goal of improving the safety and quality of services and care for the local population.
